Pre-operative Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, heart murmur, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any prescription medication (i.e. heart medications, aspirin, anticoagulant therapy like Coumadin). Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or complicated surgical treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip from your referring dentist and any X-rays if applicable
  • A written list of medications you are presently taking (names, doses, and frequency you take them)
  • If you have medical or dental insurance, bring your insurance cards and any necessary completed forms your insurance policy might require. This will save you time and allow us to help you process any claims.
  • A medical referral slip from your primary care provider if it is required of your medical insurance plan

IMPORTANT: All patients under the age of 18 years of age should be accompanied by a parent or guardian at the consultation visit. This will allow Dr. Smith to review the procedures and expected post operative results with all those involved with your care and to obtain the legally required informed consent from a parent or legal guardian.

Anesthesia choices:

I.V. Sedation:

  • Pre-operative consultation required for medical history/physical exam
  • Nothing to eat or drink for 6 (six) hours beforehand
  • Ride from a responsible adult to your home required (no driving for 24 hours)

Nitrous Oxide (Laughing gas) and Local:

  • Pre-operative consultation may be required for complicated medical history
  • Nothing to eat or drink for 6 (six) hours beforehand
  • No driving restrictions afterwards

Local anesthesia alone:

  • Pre-operative consultation may be required for complicated medical history
  • You may eat a light meal before your procedure.
  • No driving restrictions afterwards

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary or no x-ray films are available, they can be taken at our facility.
