Insurance Information

At White Mountain Oral Surgery we make every effort to provide you with the finest surgical care and the most convenient financial options. Our staff will work hand in hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information including the actual insurance cards with you to the consultation so that we can expedite your reimbursement.

We participate with following insurance organizations:

• Delta Dental
• NH Medicaid
• Medicare
• Anthem Blue Cross

We are not able to participate with all insurance plans, although you may still be eligible for coverage from them. They will require you to obtain an Out-of-Network referral in order to receive benefits for covered procedures.

Medical Insurance Referrals:

• Obtain a Medical referral from your primary care provider for most medical insurance plans.
• Referral form should be brought to your first appointment as your primary care provider is not allowed to back date these referral forms.
• You will be denied insurance benefits for any charges generated if the referral is not dated for the day the charges are generated.

For those patients eligible for Medicare:

• Medicare does not cover dental procedures.
• Specifically, Medicare does not cover the extraction of any erupted tooth, any broken tooth, any impacted tooth, or any surgery to prepare the jaws for dentures.
• Medicare does not cover dental procedures, so it does not cover the costs of hospitalization or anesthesia for any non-covered dental procedure (even if your medical condition requires treatment in the hospital).
• Medicare does cover biopsy procedures (diagnosis for pathology), surgery to remove most forms of pathology, and surgery to repair traumatic injuries from an accident such as a laceration or fractured jaw.
• The specifics of your individual coverage will be determined and reviewed at your pre-operative consultation.
